色々な国の英語に慣れることを考慮してお薦めのもの5つ挙げておく。先日の投稿「英語学習のための1ダースの特選音源」と合わせて利用するといいかも。
  • NEJM This Week:言わずと知れたNEJMのポッドキャスト。
  • The Lancet News:英国の医学雑誌"The Lancet"のポッドキャスト。
  • Australian Family Physician audio:The Royal Australian College of General Practitionersの雑誌"Australian Family Physician"のポッドキャスト。
  • White Coat, Black Art:TEDでのスピーチ"Doctors make mistakes. Can we talk about that?"で有名になったBrian Goldmanがパーソナリティを務めるカナダのラジオ番組のポッドキャスト。
  • Cross-Cultural Seminar:オリンパスの提供ということで医療関連というのは強引か?大杉正明先生とスーザン岩本さんによる英会話講座全211回。文字化されたものが入用であれば、図書化されているのもいい。

Spot Diagnosis

タイトルは、"Snap Diagnosis"と同意だが、"café-au-lait spot"と掛けたのでしょうね。Pinterestに"Medicine - Spot diagnosis"のボードがありました。
A CLINICAL PROBLEM-SOLVING article by Zornitza Stark from Murdoch Childrens Research Institute, Parkville, Victoria, Australia.
An 8-year-old girl was brought to the emergency department after she had a 10-minute episode of incomprehensible speech and blank staring, as well as a 30-second “collapse” associated with a generalized tonic–clonic seizure. MRI of the brain showed a space-occupying lesion in the left superior temporal gyrus. Neurosurgical resection of the lesion confirmed the diagnosis of glioblastoma multiforme. Despite the poor prognosis typically associated with glioblastoma multiforme, she remained well, and over subsequent years, more than 100 lentigines and two café-au-lait macules developed . The lentigines were distributed primarily over the patient's face, trunk, and upper arms.
Pigmented skin lesions are common, but given the previous diagnosis of glioblastoma multiforme, the presence of multiple lentigines and café-au-lait macules in this case raises the suspicion of an underlying systemic disorder.
The family history revealed that the patient was the youngest of three siblings. The brother who was now 17 years of age and had a history of precursor B-cell acute lymphoblastic leukemia, was found to have one café-au-lait macule and multiple lentigines with a distribution pattern that was similar to that of the patient. In this case, a clinical scenario is described in which skin findings were the clue to a familial cancer-predisposition disorder and led to the early diagnosis of a potentially fatal disease in a sibling of the index patient. 昨日、話題に上げた "Clinical Practice" のシリーズ、最新のトピックは、"Hoarding Disorder"(ため込み障害)、所謂ゴミ屋敷でした。このシリーズはすべて音声化されているのが特徴ですが、実は、右本で仲田和正先生が34編の要約を作成してくれています。ですから、このシリーズを読んだり聞いたりする場合には、下記の目次のうち太字のものを除外することをお勧めします。時は金なり、この34編のためだけにかっても1編あたり缶コーヒーより安いのですから。

英語学習のための1ダースの特選音源

先日の投稿で、自分に関わりのあるものから着手するという原則が挙げられていたので、私の関心にマッチし、Transcriptが存在するものを集めてみました。
  1. Chris Lonsdaleの講演:まず、昨日御紹介した講演をと。
  2. Brian GoldmanのTEDでの講演:TEDのプレイリスト "What doctors worry about" には、現在9つの講演が収められているが、どれも素晴らしい。Brian Goldman先生は、カナダで "White coat, Black art"というラジオ番組をもつ医師で、医師ならばもつ「心の墓場」を感動的に言語化してくれている。右本の参考文献にもこの先生の著作が挙げられています。
  3. Steve Jobsのスタンフォード大での講演:スピーチと言えば、これだけは外せませんね。銅像は酷いみたいですけど…
  4. NEJMのポッドキャスト(2012年2月12日):最新のポッドキャストを聴けば良いのだが、たまたま以前この号のトランスクリプトが入手できたので。
  5. NEJMのClinical Practice "Transverse Myelitis":随時興味に応じて、"Clinical Practice"のサイトから興味のある分野をダウンロードするといいかも。
  6. The Word Brain:ネイティヴではないスピーカの英語にも慣れたほうがいいかもしれません。
  7. Japan The Strange Country:ここからは、YouTubeから選んでみました。MediaHumanの"YouTube to mp3 Converter"などを使うと簡単にmp3音声に変換できます。動画の日本語版はこちら。まずは、より良い日本を作るために、変なところを知っておくことが大切。
  8. ER Season 1 Episode 1 - Carter and Greene:ドラマの英語は非常に難しいですが、挑戦してみましょう。まずは、第1話の落ち込んだカーターをグリーン先生が励ますシーン。後にカーターがガラントに対し同じ役割をはたすことから象徴的なシーン。
  9. ER - The news of Dr. Greene's Death:グリーン先生の死ぬ間際の手紙をカーターが読み上げるシーン。そしてグリーン先生が亡くなるときに反抗期だったレイチェルが最終回には医学生としてカーターの指導を受けることになる。
  10. 掛け算の九九:数字に関しては別途意識的にトレーニングしないと、反射的に出てきませんね。Rのスクリプトで好きな数列を作って、Free Online TTSで音声に変換してやれば、簡単に音源が作成できます。
  11. 11〜44までの二乗:英語と計算力アップの一石二鳥を狙います。何故、44までかというと、なんぼでもいいんですけど、44の二乗が1936だから。この年は、226事変、ラインラント進駐、ベルリンオリンピック、スペイン内戦、西安事件…暗雲急を告げる年でした。
  12. "Let it go"の男性版カラオケ:自分でも口ずさめるように男性版にしましたが、聴いて、感動したいならMay J、癒されたいならSarah Alainnがお薦め。

How to learn any language in six months: Chris Lonsdale


魔法のような方法の「5つの原則」と「7つの実践」をTEDの講演から抜き出すと下の通り。演者は、ウィキペディアの載るくらいの人らしい。

フォーカス予定の素材

時に幼い頃に聞いた曲を口ずさむことがある。考えてみるとそんな歌は、テレビの主題歌、音楽の授業などで時間を置いて繰り返し歌ったものが多い。結局のところ、SLAにおいてもそれは当てはまり、フォーカスした内容を繰り返すことも重要なんではないかと思った次第。「クリニカル・リーズニング・ラーニング」で取り上げられている下記4編について、精読を考えていきたい。(この名著が、英語以外に日本語とスペイン語にしか訳されていない意味を考え、感謝しながら…)

成功者とサイコパスと臨床家

今朝、出掛けにTVで「サイコパス」になりがちな職業というのをやっていたので、列記してみる。

ということらしいが、調べてみると、ソースは、"Kevin Dutton"という心理学者の調査結果らしい。眉唾だが、CEOに多いというのも頷けるかなという風くらいで考えていたら、職場で "The Five Cognitive Distortions of People Who Get Stuff Done "(「大業を成す人々の5つの認知の歪み」)という資料に出くわした。シンクロニシティを感じてメモをしておく。
1. Personal exceptionalism
2. Dichotomous thinking
3. Correct overgeneralization
4. Blank canvas thinking
5. Schumpeterianism
5番目のものはわかりづらいが、「創造的破壊」のことらしい。やはり、アウトライアーたる成功者にはそれなりの逸脱が必要なのでしょう。しかし、成功者は千三つの世界をサバイバルして大穴を当てる人たちのこと。正診率が千三つでは話になりません。(いくら誤診しようとも未発見の病気を探すという臨床家ならば別ですが…)臨床推論のコンテキストにおいては、自信過剰、割り切り、過度な一般化は、誤審の温床になってしまう。唯一、枠に囚われず考える(Think outside the box)ことが、共有すべき「認知の歪み」という結論になりましょうか。

The First 20 Hours

医学界新聞で岩田健太郎教授が「ジェネラリスト宣言」という連載を寄稿しているが、今回は『ジェネラリストの「無知の体系」』についてだった。そこに「アウトライヤー」という言葉が出てきて、思い出したのが、グラッドウェル・マルコムの「天才! 成功する人々の法則」という本。原題が、"outliers" なのでした。統計学的な意味では、外れ値、規格外、3σ以上、偏差値80以上、ロングテール…、そんな存在になるためには、最低その分野で1万時間のトレーニングが必要条件になるということを書いた本。そして、この本に対するアンチテーゼというのが、右の本。別に成功者にならなくても、そこそこやるくらいなら、20時間でいいんじゃないという主張だ。プロの2%の努力で、売りにはならなくても楽しむ分には十分でしょという考えだ。ただ漠然と20時間ではなく、その具体的な十箇条を前半で指摘している。

  1. Choose a lovable project.
  2. Focus your energy on one skill at a time. 
  3. Define your target performance level.
  4. Deconstruct the skill into subskills. 
  5. Obtain critical tools.
  6. Eliminate barriers to practice.
  7. Make dedicated time for practice. 
  8. Create fast feedback loops. 
  9. Practice by the clock in short bursts. 
  10. Emphasize quantity and speed.
著者は、TEDでも講演をしている。



時間の制約もあり、こちらで紹介しているのは、下の4箇条だ。
  1. Deconstruct the skill.
  2. Learn enough to self-correct.
  3. Remove practice barriers.
  4. Practice at least 20 hours.
著者の言うことはもっともだが、問題は最初の20時間に学ぶべきコンテンツを嗅ぎ分ける嗅覚だ。やはり凡人は、遠回りしてその内容が分かってくるのだと思うが、ネット上にそのような良質のコンテンツが蓄積されると、将来的には変わってくるのかもしれないが。

Slash Reading

どこに出掛けても、"Let it go"を耳にする今日この頃、右本を手にとると、「スラッシュ・リーディング」が紹介されていました。同時通訳者の訓練の一つで、文章にスラッシュをいれながら、帰り読みせずに文頭から理解していくトレーニング法だそうです。どういうところにスラッシュを入れるかは、「英文読解の理論と技法 染谷 泰正/増澤 洋一」に詳しいです。引用しましょう。
スラッシュを入れるのはあくまでも理解の補助のためであって、スラッシュを入れること自体が目的になってしまうのは本末転倒です。したがって、仮に、上記の原則に従えば当然スラッシュを入れるべき箇所であっても、そのままでごく自然に理解できるものはそのままにしておいてかまいません。また、あまり細かくスラッシュを入れすぎると、かえって読みやすさを損なうことになるので注意してください。ちなみに、経験的に言えば、ひとつのユニットの長さの上限はおよそ 7±2 語程度を目安にするのがいいと思います。
(1) 主語の後(=主語と動詞・助動詞の境界) スラッシュを入れるのは、原則として 3 語またはそれ以上の長さを持った重主語 (heavy subject) の後である(=> ただし、経験的には 1 語からなる主語以外はすべて主語の後に区切りを入れたほうが分かりやすくなる)。
(2) 動詞とその補語の後
(3) 主節と従属節の境界
(4) 等位接続詞および従位接続詞の後
(5) 挿入句または挿入節の前後
(6) to 不定詞句(to 補文)の前 ただし、to 不定詞の直前に動詞要素がある場合は、原則としてその動詞要素の前で区切る(例えば structural reforms / needed / to resume のようにすると不必要に細分化することになり、かえって読みやすさを損なうことになる)。
(7) 動名詞句および分詞句の前(または後)
(8) 3 語以上の長い前置詞句の前
(9) 主題化された文頭の前置詞句の後
(10) 文頭の副詞・副詞句の後
(11) "A, B, and C" および "A or B" の構造における各ユニットの境界
(12)文末句読点の後(および文中の句読点の前後)

Where has the key gone?

 "key feature"、"pivot point"の語が出現するCPSを集めてみた。
  • A fundamental characteristic of expert diagnostic reasoning is the recognition of a key clinical finding, or "pivot point." "The Drenched Doctor"
  • The pivotal step in identifying this condition as the cause of high-output heart failure in this patient was auscultation over a surgical scar. "A Missed Connection"
  • A key feature of this case was the presence of concurrent arterial and venous thromboemboli, which can be attributed to only a small number of unifying diagnoses.

Out of the Blue

A CLINICAL PROBLEM-SOLVING article by Christopher Gibson from Brigham and Women's Hospital, Boston.
A 55-year-old man with a history of heart failure presented to the emergency department with pain and swelling of his right foot and leg, which began 3 days earlier, without any prior trauma. The swelling worsened progressively, extending to the upper leg, and was accompanied by pain in the foot and calf. On the day of presentation, he noted new dusky discoloration of the right toes. The patient's medical history was notable for obesity, paroxysmal atrial fibrillation, hypertension, and heart failure with a preserved ejection fraction. He reported an intentional weight loss of 22.7 kg (50 lb) over the preceding 6 months, which he attributed to a combination of diet and exercise. On physical examination, the legs were markedly asymmetric, and the right leg and foot were dusky and cold. Although the differential diagnosis for lower-extremity edema is broad, two features of this presentation help to narrow it: the acute onset and the asymmetric distribution. Deep venous thrombosis appears to be most likely. If a deep venous thrombosis is present, its development in a patient taking warfarin suggests either a subtherapeutic international normalized ratio or a hypercoagulable state. The patient's dramatic weight loss, which would be difficult to achieve even with vigorous dieting and exercise, raises concern about an underlying cancer, which could result in a hypercoagulable state.

Unfolding the Diagnosis

A CLINICAL PROBLEM-SOLVING article by Gadi Lalazar from Hadassah–Hebrew University Medical Center, Jerusalem.
A previously healthy, 25-year-old man was admitted to the hospital because of abdominal pain, nausea, vomiting, and weight loss. Two weeks before his admission, fever (40°C), chills, and weakness developed. To control his fever, the patient ingested ibuprofen at a dose of 400 mg four times a day for more than a week. Subsequently, abdominal discomfort and nausea developed, and he presented to the emergency department owing to worsening of epigastric pain and vomiting. The physical examination was consistent with dehydration. The temperature was 36°C. The spleen and liver were mildly enlarged. Laboratory tests revealed an elevated white-cell count, elevated liver enzyme levels and albumin level of 1.4 g per deciliter (normal range, 3.5 to 5.0). Viral serologic testing was positive for IgM antibodies to cytomegalovirus.
This patient presented with a clinical picture of viral infection and severe epigastric pain. Because he had ingested high doses of NSAIDs, NSAID-induced gastritis was initially suspected as the cause of his abdominal pain. However, the finding of severe hypoalbuminemia was not easily explained by either viral infection or his NSAID use. The identification of this abnormality led to reconsideration of the working diagnosis and a shift from a “probability approach” to a “causal reasoning” approach.

Six stress accent rules

【法則1】基本的に第1音節に強勢を置く。例外は、他に強勢を置く理由がある場合。(第一音節にアクセントを置くのは、ドイツ語やオランダ語同様、ゲルマン系言語の特徴、ラテン系言語ではpaenultima中心に後ろに置かれる。)

【法則2】その「理由」とは接尾辞 (例: -ity, -ionなど) や接頭辞(例: con-, dis-, ex-, in-など)。

【法則3】接尾辞が "i" か "u" で始まれば、強勢の位置に影響する。
接尾辞の例: -ion, -ual, -ial, -ient, -ious, -ior,  -ic, -ity, etc.
強勢は接尾辞の前の音節に来る。
例: Atlantic, comic, sufficient, explanation, residual.
この法則の例外はわずかである。

【法則4】他の接尾辞は強勢に影響を与えない。
接尾辞の例: -al, -ous, -ly, -er, -ed, -ist, -ing, -ment
例: Permanent,  permanently, develop, development

【法則5】二音節の語では、接頭辞自体には通常強勢は置かれない。名詞や形容詞では例外もある。
例: To ex'pand, to de'fend; but an 'expert, a report.
接頭辞で始まる二音節の名詞は個別に覚える必要がある。
名詞と動詞が同形の場合:名前動後の傾向がある。
例外: 'access de'sign re'spect 'witness

三音節の名詞及び形容詞では、接頭辞自体に強勢が置かれる。動詞では必ずしもそうではない。
例:  'Continent, 'incident, 'exercise; to con'sider, to en'visage but to 'indicate
("-ate"で終わる三音節の動詞は、すべて第一音節に強勢が置かれる。)

【法則6】法則3は他の法則に優先される。特に、法則3の接尾辞のあとに法則4の接尾辞があとに来る場合。
例: perpetually, deliciously, conditional, conditioner, illusionist.

おすすめ英英辞典

リーディングは、12分36秒。

最近は、少しずつ英英辞典を使う機会も増えてきた。いろいろ使ってみて、それぞれ一長一短だが、お気に入りは、下の3つくらいに絞られてきた。
  1. Oxford Advanced Learner's Dictionary:英語、米語の発音をそれぞれ聴けるのが特徴。
  2. Merriam-Webster Online:語源の記載までがあるのは、便利。語源だけなら、"Online Etymology Dictionary" にはかないませんが。
  3. HyperDictionary.com:「夢辞典」がある遊び心が憎い。

文献管理ソフト"ReadCube"

文献管理は"Mendeley"を使用していた。とは言っても最近はNEJMのCase Siriesがほとんどなんだけど…。でも、"ReadCube"も評判が良いので試してみた。
結論から言えば、それぞれ一長一短で併用するのが、便利のような気がします。
文献の検索自体は、ReadCubeで行うのが便利。エンジンはGoogle Scholarも使えるので、通常手に入らない掘り出しものが手に入ることがありますし、Recommendation機能が秀逸。ボタン一つでPDFがダウンロードできます。しかし、PDFファイルのリネームが出来ないので、保存フォルダーをMendely側のWatched Foldersと共通にして、File Organizerを設定すれば、自動的にリネームもしてくれます。さらに、DropboxとWappwolfをかませれば、kindleに自動配信も出来ます。私にとっての一番の難点は、Adobe Airで開発されているようで、Linux版がないことかな。
まとめれば、文献を探し、ダウンロードするのには、ReadCube、整理にはMendeleyといったとこでしょうか。
話は変わるが、Case ReportのOpenAccess Journalをメモしておく。

Making Sense of Data

今日は、GoogleのMOOC "Making Sense of Data" を受講し、一気に終了。3月28日に最終ユニットと試験があるようです。ちょっと講義のナヴィゲーションが分かりづらかったのが難点かな。講義内容は、Googleのサービス "Fusion Tables" の宣伝といった感も否めませんでしたが、Geospational Analysisも容易なこのツールは、疫学調査などにはもってこいです。t検定とかχ2検定などの簡単な統計関数が加われば、プラットフォームがWindowsに限られるCDCのEpiInfoなどの代替アプリになり得るでしょう。もし、ジョン・スノウの時代にあったら、どれだけの命が救えたのでしょうとか、空想するのでした。ジョン・スノウに関しては、TEDで感染地図の著者が講演をしております。


講義内容とは関係ないのですが、詰まるところ、臨床推論とは、自分の経験の記憶にSearch & Filter をかけることなんではないかと思った次第。

MNEMONICS "I'M VINDICATED"

おそらくは、臨床医学でもっとも実用的なVINDICATEの語呂合わせは、英語版Wikipediaに記載があるほどに有名なのだが、なぜか過去のNEJMのClinical Caseの連載では登場したことがない。ちなみに私は、I'M VINDICATED (Iatrogenic, Metabolic, Vascular, Infection, Neoplastic, Drugs, Inflammatory, Congenital, Autoimmune, Trauma, Endocrine, iDiopathic)で覚えています。VINDICATE以外でも、語呂合わせが引き合いに出される例は少なく、CPCの"Case 9-1984"では、低血糖の鑑別の語呂合わせ"EXPLAIN"が紹介されております。
As a first approximation, the mnemonic that I use for the differential diagnosis of fasting hypoglycemia in an adult is based on the word "EXPLAIN." "EX" refers to exogenous drugs, such as insulin, sulfonylureas, and alcohol. "P" indicates pituitary insufficiency. "L" denotes liver failure and a variety of enzyme deficiencies that affect hepatic glycogenolysis and gluconeogenesis. "A" is for adrenal insufficiency. "I" initially represented islet-cell tumors or hyperplasia but with the description of two new syndromes it also stands for immune-mediated hypoglycemia associated with anti-insulin receptor antibodies and spontaneously occurring anti-insulin antibodies. We more commonly associate such antibodies with insulin resistance, but antibodies of both types have been accompanied by severe hypoglycemia. In vitro antireceptor antibodies are acutely insulinomimetic, which may explain their ability to produce hypoglycemia. "N" refers to neoplasms, nonpancreatic.

A CLINICAL PROBLEM-SOLVING article by Ole-Petter Hamnvik from Brigham and Women's Hospital, Boston.
A 40-year-old woman presented to the emergency department with pain, weakness, and poor oral intake. In the 3 months preceding presentation, she noticed the gradual onset of bilateral diffuse flank pain that progressed to affect her lower back, abdomen, and both arms and legs. She eventually became homebound. The patient reported poor appetite and reduced food intake and had lost approximately 4 kg. Nausea developed 2 days before presentation, and the patient became unable to take anything by mouth, which prompted her to seek evaluation.
The physical examination revealed a cachectic, ill-appearing woman in distress from pain. The medical history included an immature teratoma, which had been treated 10 years before presentation and a history of chronic infection with hepatitis B virus. Because the infection did not respond adequately to the prior regimens, treatment had been changed to tenofovir 10 months earlier.
Laboratory results revealed severe hypokalemia. A bone scan showed multiple abnormal areas with increased tracer uptake, particularly in the left humerus, the right femoral neck, and the bony thorax.
This patient had a dramatic presentation, which raised the suspicion for metastatic cancer. Although the findings on the bone scan strengthened this suspicion, several metabolic abnormalities — including profound hypokalemia and metabolic acidosis — were not consistent with this diagnosis.

Missing Elements of the History

A CLINICAL PROBLEM-SOLVING article by Larry Allen from University of Colorado School of Medicine, Aurora.
A 59-year-old woman with a history of bilateral total hip replacements and a total knee replacement sees her physician for cough, exertional dyspnea, and foot swelling that had developed 2 weeks earlier while she was on vacation in Denmark. She received a diagnosis of pneumonia and “travel-related edema” and was treated with a course of antibiotics. Over the next week, the patient had progressive dyspnea, edema, and fatigue.
CT of the chest showed cardiomegaly and changes consistent with pulmonary edema; these findings prompted admission to her local hospital. Her jugular venous pressure was 13 cm of water, with normal respiratory variation. Cardiac examination revealed a regular rhythm, normal S1 and S2 sounds, an S3 gallop, and a grade 2/6 holosystolic murmur across the precordium. Cardiac catheterization ruled out coronary disease as the cause of new-onset heart failure. Treatment with furosemide, carvedilol, and lisinopril was initiated.
Despite some initial improvement in symptoms, six months after the onset of symptoms, she was having difficulty climbing one flight of stairs because of progressive dyspnea. Repeat echocardiography showed increased pericardial effusion.
As this case progressed, unusual causes of heart failure were considered. Additionally, the patient learned that her metal-on-metal hip implant was recalled.
